CCW Services Available Through Enable

Behavior Supports, Community Transition, Day Habilitation, Individual Supports, Prevocational Training, Respite, and Transportation are all now available to be provided by Enable through the NJ Division of Developmental Disabilities (DDD) Community Care Waiver. CCW covers the services and supports individuals with developmental disabilities need to live in group home settings in the community – although in some cases, individuals with other living arrangements may qualify. DDD administers the program with funding from the State and federal Medicaid program. Enable works with the DDD to establish whether individuals qualify for CCW by meeting these requirements:

  • Eligible for DDD-funded services
  • Meet the level of care for Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/ID)
  • Meet Medicaid requirements regarding income and resources 

CCW services to be provided by Enable:

  • Individual Supports. Enable’s experienced staff supports participants in achieving increased independence, productivity, enhanced family functioning, and inclusion in the community. We support community-based activities, and train and supervise individuals on basic self-care, social interaction, and activities of daily living.
  • Behavior Supports. Enable’s highly qualified behaviorists assess each individual’s challenging behavior, develop a structured behavior support plan, manage plan implementation, train and supervise direct support staff and family caregivers, and periodically reassess the plan.
  • Day Habilitation. Certified Day Habilitation services are offered at Enable’s Roszel Day Services (Mercer County), Victorian Day Services (Hunterdon County), and Fusion Day Services (Middlesex County). The programs focus on education and training. Skills are acquired through participation in community based activities which also build positive social behavior and interpersonal confidence.
  • Prevocational Training. Enable offers learning and work experiences that develop general, non-job-task-specific strengths and skills. The goal is to build employability in paid employment in integrated community settings.
  • Community Transition Services. We facilitate the transition from institutional or other group living arrangements to less restrictive living arrangements. Also included is transition to a private residence where the person is directly responsible for his or her own living expenses. Many transitioning costs are covered.
  • Respite. Enable Respite Services provide short-term care for individuals who are not able to care for themselves because their normal caregivers are absent or need relief. Hours, duration and location of respite options are flexible. Respite is provided in the participant’s home, a group home, or another DHS approved community based setting.
  • Transportation. Enable provides transportation so participants may access services, activities and resources specified by their Individual Service Plan.
