In Home Supports

As we get older, sometimes the best help comes simply from having an extra set of hands around the house. Enable provides a host of in-home services to seniors who are living on their own or independently with their families, from a few hours each day to a hundred hours each week. Enable’s professional, experienced caregivers assist seniors with private, personal services that maintain the dignity of those who are unable to care for themselves.

 

We work with individuals, married couples, parents with children, and others on a regular basis, providing over 35,000 hours of in home care each year. Self-pay options are available, as well as Medicaid, Medicare, and third-party and long-term care insurance. Enable’s In Home Support services include:

 

Personal Care

 

Our compassionate assistance with personal care maintains the dignity of seniors who can’t manage on their own. Our caring team assists with a range of daily living activities such as dressing, bathing, grooming, toileting, eating, walking, functional mobility (moving from place to place), and more.

 

Help with Chores

 

It takes a lot of work to keep a household running smoothly. Enable’s experienced, trustworthy staff assists with a variety of chores to help maintain a quality standard of living and a positive home environment for your loved one. This may include gardening and housework, grocery shopping, diet & nutrition support, help with vitamins and medications, mailing bills and letters, and more.

 

Transportation

 

Transportation is a key issue for older adults. Maybe they find it hard to drive, or don’t like to drive at night. Whatever the concern, having the freedom to get around helps seniors keep their independence, make important appointments, and maintain their social network. Enable provides reliable, trustworthy transportation options to help seniors get to medical appointments, go shopping for groceries or personal items, visit with friends, attend events, and more.

 

Respite

 

Welcome to the sandwich generation: thousands of adults are caring for their aging parents while juggling their own family, work, and personal lives. When an older loved one requires more intensive care, sometimes around the clock, it’s these informal caregivers who are often overwhelmed. In this situation, taking care of yourself is as important as taking care of your loved ones.

 

Let Enable help. Our compassionate and experienced staff offers respite care to provide caregivers with an average of 20 hours each month to take the time they need – whether it’s to go to work, run errands, or just get some R&R. Enable works directly with caregivers like you to design a program for respite care that includes both in-home and out-of-home options. Emergency respite care is also available for last minute or unanticipated demands.

Mercer County Respite care options include:

Mercer County Statewide Respite Program

  • A statewide respite program, began in 1988.
  • Provides relief and support to family or other uncompensated caregivers from the demands of the daily care of persons with physical and cognitive challenges, including the frail elderly.
  • Limited to short-term intermittent care such as:
    • Adult Day Health Services
    • Alternate Family Care
    • Assisted Living
    • Campership
    • Companion Services
    • Homemaker/Home Health Aide
    • Nursing Facility Respite
    • Private Duty Nursing
    • Social Adult Day Care
    • Residential Health Care
    • Facility Respite
    • Hospital Inpatient Respite

Eligibility for this program:

  • 18 years or older, functionally impaired.
  • Cared for at home by a caregiver who is not compensated for his or her services.
  • At risk of long-term institutional placement if his or her regular caregiver is unable to continue in his or her role.
  • Must be NJ resident.
  • Financial eligibility.*

The Jersey Assistance for Community Caregiving (JACC)

  • State funded program that provides services to older individuals who meet nursing facility levels of care, but are not eligible for Medicaid or Medicaid waiver services.
  • Provides stability in their own homes.
  • Designed to maximize participants’ autonomy in managing and directing his or her services.
  • Services include:
    • Care Management
    • Adult Day Health
    • Chore Services
    • Personal Care
    • Personal Emergency Response System
    • Respite Care
    • Social Adult Day Care
    • SME & Supplies
    • Transportation

Eligibility for this program:

  • Resident of the State of New Jersey.
  • United States Citizen or a qualified alien.
  • 60 years of age or older.
  • Resides in a home — not a facility licensed by the state.
  • Clinical eligibility requirements.+
  • Meets financial eligibility requirements.*
  • Financially ineligible for services provided through New Jersey Family Care.
  • Has no alternate means to secure the services and/or supports needed to remain in the community.
  • Does not receive services from any other State funded home-based supportive care program.
