Enable staff connect persons who may have disabilities due to aging or illness with a variety of services. Care management is offered as one of several services available through two collaborations funded by the United Way of Greater Mercer. Through the collaboration partners, persons served may access a wide variety of services which include counseling, in-home nurse visits, meals at home, transportation, and volunteer visitors.
