Enable staff connect persons with disabilities (including those who may have disabilities due to aging or illness) with a variety of services. Care management is offered as one of several services available through two collaborations called Partners in Caring and Project Connect. Other services include counseling, in-home nurse visits, meals at home, transportation, and volunteer visitors.
