Enable Provides DDD Supports Program Services

Enable is qualified by the New Jersey Division of Developmental Disabilities (DDD) and Medicaid to provide Supports Program Waiver services to consumers. The Supports Program ensures that adult individuals over 21 years of age who are Medicaid eligible and live with their families or in other unlicensed settings receive needed supports and services. The services are determined during a person-centered planning process and documented in their individual service plan (ISP).

Enable is qualified to provide Supports Program services including Behavioral Supports, Community Based Supports, Day Habilitation, Prevocational Training, Respite, and Transportation.

 

  • Behavioral Supports. Positive behavior supports improve quality of life by reducing challenging behaviors and increasing more socially appropriate behaviors. Enable’s highly qualified and experienced behaviorists assess the individual’s challenging behavior, develop a structured behavior support plan, manage plan implementation, train and supervise direct support staff and family caregivers and periodically reassess the plan.
  • Community Based Supports. Our caring and experienced direct care staff provide direct support and assistance to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community. Services are delivered in or out of the home.
  • Day Habilitation. Enable offers day habilitation at Roszel Day Services in Mercer County, Victorian Day Services in Hunterdon County, and Fusion Day Services in Middlesex County. The focus is on education and training to acquire the skills and experience needed to participate in the community. Our day service environment fosters skill acquisition and greater independence and personal choice, while building positive social behavior and interpersonal confidence.
  • Prevocational Training. Enable offers learning and work experiences, which develop general, non-job-task-specific strengths and skills. The goal is to build employability in paid employment in integrated community settings.
  • Respite. Enable’s in home respite provides short-term care for individuals who are not able to care for themselves because their normal caregivers are absent or need relief. Hours, duration and location of respite options are flexible.
  • Transportation. Enable provides transportation so participants may access services, activities and resources specified by their ISP.

 

DDD is rolling out the Supports Program over a series of months. Individuals are notified when they are identified to be enrolled.
